Q: Do you take insurance? A: At this time, we do not accept insurance. Payments are due at time of visit.

A: We will provide you with an itemized superbill that will include appropriate diagnostic (ICD) and treatment (CPT) codes that you can submit to your insurance company for reimbursement after you have made payment to our office. Many insurers provide reimbursements for an “out-of-network” provider but this will vary depending on your specific plan. If you have a PPO insurance plan and have met your deductible, it is likely that a large percentage of the charges will be reimbursed to you at the out-of-network rate. You can usually obtain claim submission forms on the website of your insurance carrier. In addition, services may be applied towards qualified medical expenses if you have a Health Reimbursement Account (HRA) through your employer or Health Savings Account (HSA).
